How do I apply to Copernicus Lodge Long-Term Care?

The long-term care home placement process includes a multi-part assessment, including an assessment by a care coordinator to ensure there’s an accurate record of your medical history and to help determine your eligibility for long-term care.

Your first call to Ontario Health atHome will determine whether a care coordinator should visit you for an in-person assessment.  At this visit, the care coordinator will assess your health, and may conduct other assessments as well, including capacity assessment, functional assessment, and behavioural assessment.

Once determined eligible, your care coordinator will discuss all options, including assisting you with completing and submitting the application to our home.

Once received, Applications are then reviewed by our Admissions team, and a decision is made as to whether we can meet your needs and which care unit would be the best fit for you.

Ontario Health atHome is then notified and your name is placed for admission on the Copernicus Lodge waitlist, to be reviewed again when an appropriate bed becomes available.
